Aging Care Concierge

Aging Care Concierge aims to improve the quality of life for older adults through a range of personalized services. These include comprehensive assessments, personalized care plans, coordination of care, family guidance, caregiver support, patient advocacy, crisis intervention, end-of-life planning, and cost-effective solutions. They also provide extensive resource referrals for medical, legal, and alternative living needs, with the goal of enhancing well-being, socialization, independence, and dignity. Lorna Van Ackeren, BA, MPA, is a Certified Geriatric Care Manager at Aging Care Concierge. Lorna leverages her extensive knowledge and expertise to assist families in navigating the complexities of aging and the healthcare system. Her aim is to diminish family caregiver burnout while ensuring that loved ones receive dignified and respectful care. Lorna's goal is to guide families toward making well-informed care decisions. She has been actively involved in numerous programs serving the older community in Contra Costa County, including the Contra Costa County Advisory Council on Aging, Choice in Aging, the Commission on Aging in the City of Pleasant Hill, Meals on Wheels Contra Costa, the East Bay Veterans Community Partnership, and the California Senior Legislature.
